It is our continuing goal to provide you with the most comprehensive and up-to-date health care possible. It is important that our patients become partners with us in their own ongoing health care.

Your First Visit

If you are a new patient, you will receive an information form and a medical history form to complete upon signing into our office. You also have the option to go into our patient portal and complete the forms prior to your visit.
In order to keep a timely schedule for all of our patients, we encourage new patients to arrive a little early as well as complete your new patient forms and bring them to the appointment.
  1. Please bring a list of all medications with you that you are presently taking.
  2. New patients must bring x-rays and MRI's (if applicable) to your first office visit.

We ask that you please realize that all patients do not require the same amount of time to be seen by the doctor.
Therefore, a post-op patient or a patient scheduled for a return visit maybe be taken before a new patient or patient requiring fracture care.
We also, occasionally, have emergency conditions arise which may result in minor “in-office surgical” treatment. When this occurs, please bear with us, every patient's care is important to us.

Office Hours

Our office hours are 8:00 a.m. to 5:00 p.m. Monday through Friday. We have someone available at all times during these hours to assist you should the need arise.
After office hours and on week-ends and holidays, we have a 24-hour answering service for your safety and convenience.
Our answering service will relay your concerns to the doctor on call and he or she will return your call.
We request that office business, such as appointment scheduling, billing matters, and prescription requests be addressed during regular office hours.
