Your First Visit
If you are a new patient, you will receive medical history forms to complete upon signing into our office. You also have the option to go into our patient portal and complete these forms prior to your visit.
- Please bring a list of your current medications.
- New patients must bring x-rays and MRI's (if applicable) to your first office visit.

We ask that you please realize that all patients do not require the same amount of time to be seen by the doctor.
Therefore, a post-op patient or a patient scheduled for a return visit maybe be taken before a new patient or patient requiring fracture care.
We also, occasionally, have emergency conditions arise which may result in minor “in-office surgical” treatment. When this occurs, please bear with us, every patient's care is important to us.
